PENCIL Health provides educational content, general written care-plan templates, and workflow tools intended primarily to support licensed healthcare professionals. PENCIL Health does not practice medicine, provide patient-specific medical advice, diagnose or treat any person, prescribe treatment, supervise clinicians, or establish a clinician-patient relationship.
Not a Substitute for Clinical Judgment
PENCIL Care Plans support—but do not replace—independent professional judgment. Before giving a care plan to a patient, the healthcare professional is solely responsible for reviewing its accuracy, currency, completeness, and appropriateness for the patient’s diagnoses, medications, allergies, comorbidities, age, pregnancy status, health literacy, culture, language, preferences, local law, scope of practice, and individual circumstances.
Healthcare professionals remain responsible for diagnoses, prescriptions, treatment, informed consent, warnings, contraindications, documentation, follow-up, referrals, emergency instructions, patient communications, and all other care.
Evidence and Updates
References to content as evidence-based describe PENCIL Health’s intended editorial approach. Medical evidence, guidance, product availability, warnings, and professional standards change. PENCIL Health does not warrant that every statement is exhaustive, current at all times, error-free, or appropriate for every patient or jurisdiction. Printed and downloaded copies do not automatically update.
No Guaranteed Outcome or Standard-of-Care Determination
PENCIL Health does not guarantee patient comprehension, recall, adherence, satisfaction, outcomes, risk reduction, reimbursement, regulatory compliance, or satisfaction of a legal or professional standard of care. A PENCIL Care Plan does not replace the medical record, required disclosures, informed-consent documentation, or direct communication with a patient.
Public Visitors
Public Website content, search results, previews, and examples are general information and should not be used by patients or other members of the public to diagnose, treat, or make medical decisions. Consult an appropriate licensed healthcare professional regarding personal health concerns.
No Emergency Use
The Website and PENCIL Care Plans are not designed for emergencies, urgent communication, real-time monitoring, or crisis response. Call 911 or the applicable local emergency number and seek immediate professional care when appropriate.
International Use
Content may be developed primarily from United States sources and practices. International professionals must determine whether content is appropriate and lawful in their jurisdiction, including differences in terminology, approved products, medication labeling, professional standards, emergency systems, and patient-communication requirements.
